What we collect

When you submit the enquiry form on this site we collect your name, email address, phone number, the clinic and treatment you selected, your preferred appointment time and the message you wrote. We also record the date and time of the submission.

We do not ask for clinical details, health fund numbers, Medicare numbers or payment information through this website, and you should not send them by email. Those are collected in the clinic, where they can be stored in our practice management system under the controls that apply to health records.

How we use it

Enquiry details are used to respond to your enquiry and to arrange an appointment. They are emailed to our front desk at admin@radiantsmiles.com.au and an acknowledgement is sent back to the address you provided. We do not sell your information, and we do not add you to a marketing list from this form.

Health records

Once you become a patient, your clinical records are handled in line with the Australian Privacy Principles and the health records obligations that apply to dental practices in Western Australia. You can request access to your own records, and you can ask us to correct information that is wrong.

Cookies and analytics

This site uses a small analytics script to understand which pages people find useful. It records page views and general usage patterns. It does not identify you personally and it is not used to build an advertising profile.

Security

The site is served over HTTPS and enquiry submissions are transmitted encrypted. Access to the enquiry inbox is limited to practice staff who need it. No system is perfect, and we will tell you if a breach affects your information.

How long we keep it

Website enquiries are retained only as long as needed to respond and to make the appointment. Clinical records are retained for the periods required by law, which is generally seven years from the last entry for an adult, and until the age of 25 for a patient who was a child at the time.

Third parties

We use an email service to deliver enquiry notifications and acknowledgements, and a hosting provider to run this website. Neither uses your information for their own purposes. Health fund claiming is processed through HICAPS at the point of payment.
